DENTAL INSURANCE
IMPORTANT NOTIFICATION 
If your BCBS policy is underwritten by Dentemax, you have a NON-PPO policy
and we are NOT a preferred provider.
Our first priority is to give our patients the highest standard of dentistry available.
We are a fee for service practice.
This means although we are happy to help file your insurance, the patient is responsible for the portion of the visit the insurance company does not pay for.
If you have an indemnity plan you can go to the dentist of your choice.
You may have a policy that has a preferred provider list (PPO).
We are a provider for Guardian, Cigna, and Blue Cross PPO dental plans.
If we are not on your PPO list, you may elect to come out of network and pick Dr. Burgess.
We will file your insurance for you and the patient is responsible for the portion insurance does not pay.
